Social prescribing is a healthcare approach where healthcare providers refer patients to non-clinical community resources and programs to improve their health and well-being. It addresses social needs that impact health—like access to food, transportation, light housekeeping, and social connections. This approach shifts the focus from solely medical interventions to a more holistic view of health, recognizing that social factors play a significant role in overall well-being.


While healthcare providers are a common point of referral, many other organizations—such as pharmacies, Family and Community Support Services (FCSS), faith-based leaders, the RCMP, banks, care facilities, and other community organizations—also play a vital role in connecting individuals with the resources they need.
